About The Position

This role focuses on case management and discharge planning for patients, operating on a 40-hour per week schedule. The specific shift pattern may vary between 4x10-hour days or 5x8-hour days, all within the day shift, Monday through Friday. The position requires a Registered Nurse (RN) with a strong background in case management and a proven ability to facilitate effective patient discharge planning within an acute care hospital setting.

Requirements

• At least 2 years of case management experience.
• 6 months of discharge planning Case Management experience in an Acute Care hospital.
• Experience in a large organization/teaching hospital with 500 beds or more is ideal.

Responsibilities

• Facilitate patient discharge planning.
• Manage patient cases within an acute care hospital setting.
• Coordinate care to ensure smooth transitions for patients.
