About The Position

As a Case Management Director, the primary objective of this role is to provide leadership in overseeing the Waiver Case Management (WCM), Targeted Case Management (TCM), and Dual Case Management (DCM) ensuring the delivery of high-quality case management services. The incumbent will supervise interdisciplinary teams, offer clinical guidance to the Targeted and Waivered Case Management teams, and provide consultative support to Residential Transitions Inc. (RTI) team members. Responsibilities include approval of treatment and community support plans that align with the needs of the targeted population, maintaining consistency in treatment services, assuring statutory or contract compliance and adapting to changes in program oversight.

Requirements

  • Bachelor's degree in a behavioral health related field.
  • At least 2 years of management experience in case management.
  • Knowledge of community resources and how to access them.
  • Meets the MN statutory requirements for Dual Case Manager.
  • Must pass the required background check and remain eligible to provide services as defined by the Minnesota Department of Human Services.
  • Must pass the screening required by Federal Human Services Office of the Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.

Responsibilities

  • Supervise Targeted, Waivered, and Dual Case Management teams to ensure quality clinical services, accurate documentation, implementation of treatment plans, and compliance with State, County, and Agency standards.
  • Represent RTI in external meetings and trainings.
  • Participate in County and State-led meetings to align agency services with stakeholder expectations.
  • Coordinate with county and community stakeholders to ensure responsive and collaborative service delivery.
  • Provide leadership, training, performance feedback, and professional guidance to support team development.
  • Foster an environment of support, accountability, and continuous improvement.
  • Oversee the development and scaling of Targeted and Waivered Case Management teams.
  • Recruit and retain qualified staff to ensure capacity aligns with program growth.
  • Develop and revise case management policies, procedures, and strategic vision.
  • Lead program planning and expansion efforts, including service enhancements based on community needs.
  • Monitor scheduling of diagnostic assessments and perform regular clinical chart reviews.
  • Collaborate with the Chief Operating Officer to manage caseload distribution and ensure participant needs are met.
  • Ensure contract compliance and implementation of corrective actions when needed.
  • Provide clinical consultation and crisis support to team members.
  • Support participants through guidance on coping strategies, independent living skills, and accessing community resources.
  • Oversee referral processes and ensure timely access to necessary services.
  • Oversee development of new services and lead implementation of person-centered programming.
  • Manage the review and renewal process for county contracts.
  • Monitor and track contractual deliverables and reporting requirements.
  • Identify opportunities for new funding or service areas to support RTI’s mission.
  • Monitor judiciary requirements for all TCM participants.
  • Ensure Targeted Case Managers are trained and supported in fulfilling legal and commitment oversight responsibilities.
  • Facilitate consultation with county stakeholders on cases involving commitments.
  • Foster an environment of support for the people served by RTI.
  • Manage budgetary and financial needs of the programs effectively.
  • Track training requirements and ensure staff meet all regulatory obligations.
  • Develop and implement ongoing staff training in best practices, compliance, and case management roles.
  • Develop and implement structured onboarding for all case management roles.
  • Oversee voluntary and involuntary terminations in collaboration with RTI’s Human Resources Department.
  • Undertake additional projects as assigned
