About The Position

The BCBS Delegated Case Management Coordinator supports the Case Management (CM) and Disease Management (DM) programs by coordinating member outreach, scheduling post-hospital follow-up appointments, and maintaining program data and reporting. This role works closely with Nurse Case Managers to ensure timely engagement of members, facilitate care transitions, and support overall population health management efforts.

Requirements

  • High school diploma or GED required
  • one (1) year of experience in a medical office, hospital, or healthcare setting required
  • Knowledge of medical terminology and healthcare office operations
  • Strong communication and customer service skills
  • Effective problem-solving and decision-making abilities
  • Ability to work independently and manage multiple priorities

Responsibilities

  • Conduct member outreach to schedule post-discharge follow-up appointments and support transition of care.
  • Complete Health Risk Assessments (HRAs), identify member needs, and refer cases to Nurse Case Managers as appropriate.
  • Support Care Management and Disease Management programs through member engagement and coordination activities.
  • Maintain and track case management data, including eligibility, active census, productivity, and outcomes reporting.
  • Assist with report preparation, discharge list compilation, and data analysis for program performance.
  • Provide operational support including managing member mailings, responding to phone inquiries, and maintaining resource materials.
  • Serve as a liaison for case management systems and external portals.
  • Assist with referral processing and provide clerical support such as documentation, faxing, mailing, and medical record retrieval.
