About The Position

The Rite of Passage Team is seeking a Case Management Coordinator for Walton Academy for Growth and Change in DeFuniak Springs, FL. Walton Academy provides residential treatment, mental health, and substance abuse services to youth, offering various skills and opportunities including sports and culinary arts. The organization has a 40-year legacy of helping young people become successful adults. This role is part of the Site Management team and offers the opportunity to create a positive, safe, and supportive environment for the youth served, with career growth and self-fulfillment.

Requirements

Bachelor’s degree in related field
Four years of experience in Human Services
Prior case management experience required including three years’ experience with juveniles
Knowledge of Placing Agencies, licensing requirements and Rite of Passage policies and procedures
Ability to pass a criminal background check, drug screen, physical and TB test
Ability to effectively communicate, verbally and in writing
Must be able to maintain a high level of confidentiality
Able to work in excess of 40 hours per week as needed
Must be flexible and able to handle multiple priorities

Nice To Haves

Master’s degree preferred
Prefer two years of Rite of Passage experience
Supervisory experience preferred

Responsibilities

Overseeing all aspects of case management training
Overseeing audit compliance
Overseeing outside agency relationships, including community service, to ensure treatment continuity is maintained for the students
Creating a positive, safe and supportive environment for the youth we serve
May supervise the Case Managers, Transitional Services or other designated staff
