About The Position
Gathers all relevant data related to patient’s treatment status; objectively evaluates it to identifies barriers, clarifies realistic goals & seeks potential alternatives to care. Works with the patient/family, primary care physician & multi-disciplines to develop a treatment plan that enhances patient outcomes; facilitates communication & collaboration between the patient & health team; advocates patient empowerment in their own care.
Requirements
- RN required
- Two to three years hospital experience required
Nice To Haves
- BSN preferred
- Home Care and/or Utilization Review of Case Management experience desired
