About The Position

The Project is excited to announce that we are seeking a Case Management (CM) Manager to lead the daily operations of The Project of the Quad Cities’ (TPQC’s) case management, housing, and insurance benefit services for people living with HIV in the Quad Cities and surrounding areas. Reporting to the Director of Programs, the CM Manager is the hands-on leader who keeps case management compliant, flexible, and responsive to the needs of the community. The CM Manager leads a case management philosophy grounded in best practices, while creating space for sound judgment and client centered flexibility. This position offers a hybrid work arrangement that includes both remote work and required in-office work.

Requirements

• Bachelor’s degree in public health, social work or related fields such as psychology, behavioral health, etc.
• Minimum of four years of progressive experience in community health, public health, healthcare, or related human services, with at least two years in a supervisory, program management, or contract administration role.
• Demonstrated ability to build and sustain relationships with community partners over time, with examples of follow-through even when results weren’t immediate.
• Demonstrated attention to detail and consistency in managing workflows, documentation or compliance requirements, alongside the ability to adjust approach based on the person, setting, or situation.
• Ability to communicate effectively across all levels of the organization and with community partners and clients.
• Experience working across clinical and community-based environments, or a clear ability to translate skills between the two.

Responsibilities

• Provide direct supervision and performance oversight of the HIV case management team and the IA Benefits Case Manager.
• Manage daily operations for client services including being on the frontline for client de-escalation and addressing any client and assigned case manager obstacles and/or client grievances in conjunction with the Director of Programs.
• Act as a subject matter expert for case management standards, HOPWA, and client financial assistance requests to support staff and fulfill grant expectations.
• Ensure financial assistance provisions are allowable, reasonable, and within budget.
• Maintain a high level of working knowledge of Medicaid, Medicare, ADAP, Marketplace, and other coverage systems, serving as a resource and backup to the Benefits Case Manager when navigating complex client coverage needs.
• Collaborate with the Ryan White Illinois Region 1 Contract Manager to maintain clear role boundaries between programmatic decision-making, contract requirements, and fiscal oversight, with the Contract Manager determining allowability and the CM Manager determining client-level service decisions.
• Maintain strong working relationships with TPQC partner agencies including MercyOne Infectious Disease Clinic and Trinity UnityPoint and have a working knowledge of referral agencies and resources in the communities served.
• Supervise, mentor, and evaluate case management staff; approve schedules, timesheets, and leave requests.
• Hold assigned staff accountable for following established funder requirements, addressing gaps in training, performance, or adherence.
