About The Position

This position assists Case Management and Social Work staff with setting up patients’ discharge needs. The role involves obtaining Durable Medical Equipment (DME), making basic transportation arrangements, maintaining patient confidentiality, rounding on assigned units, and updating patient information in EPIC. The assistant will also utilize educational opportunities to improve skills and project a positive image of the department and hospital.

Requirements

• HS diploma or equivalent and 3-5 years’ experience.
• Proficient with computer/software applications.
• Strong written and verbal communication skills and organizational skills.
• Must be able to multi-task and prioritize.

Responsibilities

• Assists with obtaining Durable Medical Equipment (DME), including but not limited to rolling walkers, commodes, wheelchairs, quad canes and hospital beds.
• Make basic transportation arrangements (MAS, Rehab, back transfer, cabs) once discharge planner has documented need.
• Maintains patient confidentiality of all communications and documents as required by Hospital policy.
• Rounds frequently on assigned units.
• Keeps Case Managers and Social Workers apprised of status of discharge plans, documents the status of activities and communications, and maintains adequate reporting information in EPIC.
• Enters data into EPIC in a timely fashion to facilitate discharge plans.
• Maintains skills needed for performance of duties.
• Utilizes available educational opportunities for improving skills.
• Attends required in house in-services.
• Projects positive image of the department and hospital.
• Assist in the delivery of regulatory documents to patients.
