About The Position

This Analyst, Case Management Field position is with Aetna’s Long Term Services & Supports (LTSS) team and is field-based in Chicago, Illinois. The analyst manages a full caseload of waiver members and facilitates appropriate healthcare outcomes by providing care coordination, support, and education. The role requires in-person home visits with members and is critical to meeting contractual requirements. Candidates must travel 50–75% of the time to meet members face to face. Mileage is reimbursed under the company’s expense reimbursement policy. The position is associated with the listed Chicago ZIP codes and surrounding areas.

Requirements

At least two years of case management experience.
Experience with Microsoft Office and electronic health records.
Reliable transportation and willingness and ability to travel up to 75% of the time.
Valid Illinois driver’s license.
Must reside in one of the listed ZIP codes or surrounding areas: 60607, 60608, 60612, 60622, 60623, 60624, 60639, 60642, 60644, 60647, or 60651.
A bachelor’s degree in a human-services field, or a master’s degree in a human-services field for a non-licensed individual. Relevant fields include sociology, special education, and rehabilitation counseling.
