Case Management Analyst - Field in Northern Virginia

Aetna’s Long‑Term Services and Supports (LTSS) program is looking for a passionate, member‑focused Case Management Analyst to support individuals and families across the Northern Virginia region. This is a field‑based role where you’ll spend meaningful time face‑to‑face with members—listening, advocating, and helping them navigate complex health and social needs. Requirements
- 2+ years of experience in behavioral health, social services, or a related field aligned with program focus.
- Must reside in Northern Virginia in one of these locations; Arlington, Annandale, Falls Church or Springfield
- Must possess reliable transportation and be willing and able to travel up to 75% of the time.
- Mileage is reimbursed per our company expense reimbursement policy
Nice To Haves
- Experience in case management or discharge planning.
- Managed Care experience.
Responsibilities
- Build trusted relationships with members through in‑person visits (approximately 75% travel).
- Conduct comprehensive assessments using clinical judgment and critical thinking to understand medical, behavioral, and social needs.
- Develop and implement individualized care plans that promote health, independence, and quality of life.
- Collaborate closely with providers, community resources, and interdisciplinary teams to ensure coordinated, high‑quality care.
- Connect members to services, supports, and resources that align with their goals.
- Ensure care delivery meets regulatory requirements and Aetna quality standards.
- Advocate for medical appropriateness while supporting positive member engagement and outcomes.
