The Case Manager is responsible for an assigned caseload of Toby’s Place residents, helping them transition into the program, providing encouragement and accountability, and assisting them in overcoming barriers to stability, recovery, independence, and sustainable transition. The Case Manager develops professional helping relationships with residents and facilitates person-centered assessment, planning, service coordination, resource navigation, advocacy, progress monitoring, and transition planning. The Case Manager works collaboratively with residents, the Care Team, Spiritual Formation, Residential Support, Intake & Program Access, community resources, and other service providers to support individualized goals and coordinated care. The Case Manager will maintain accurate and confidential case documentation, monitor resident progress, identify changing needs and barriers, and ensure appropriate follow-through on referrals and services. The residents remain active participant in defining their goals, priorities, preferences, and desired outcomes. The Case Manager may be required to participate in on-call duties and some evening and weekend work as assigned.
