Summary: The Case Coordinator plays a crucial role in helping individuals achieve physical and mental wellness by planning and coordinating health and social services. Responsibilities include advocacy, assessment, planning, and education. The Case Coordinator, in collaboration with the care team, ensures that individuals are effectively connected with services in a coordinated manner. Additionally, they provide ongoing support and monitor each individual's progress throughout their treatment journey.

Requirements

  • High School Diploma or equivalent required.

Nice To Haves

  • 1 year of experience in a drug and alcohol or mental health setting.
  • Valid Driver’s License.
  • CPR/B certification preferred.

Responsibilities

  • Coordinate client engagement through appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
  • Maintain communication with referral sources and external providers, ensuring consistent, professional interactions and thorough documentation at admission and throughout treatment, including UR, legal, and employment needs.
  • Provide case coordination services by addressing clients' transportation needs.
  • Conduct individual case coordination sessions to support client progress.
  • Assist in developing aftercare plans, including referrals and contact information for aftercare providers, while addressing outside provider needs.
  • Facilitate groups to support clinical services and engage clients in the program on a weekly basis.
  • Collaborate with the treatment team and maintain professional communication with counselors and other team members.
  • Document client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines.
  • Work with the client navigation team to address client aftercare needs.
  • Provide onsite support for Enhanced Outpatient Sessions (EOS) appointments between clients and the Client Navigation team.
  • Participate in mandatory corporate training, program training, and professional development.
  • Perform other duties as assigned.
