This position is responsible for providing case management services and maintaining a caseload of Long Term Support Clients, supporting them in a variety of job sites. The role requires an understanding of realistic work expectations regarding quality of work and appropriate behavior. The Case Coordinator will act as a liaison between employers and employment services staff, as well as the individual's care team, including funding sources, employers, and families/caregivers. The ability to work independently and creatively problem-solve as needs arise for individuals is essential.
