About The Position

The Ohio Children's Alliance is seeking a highly organized and detail-oriented Case Coordinator for the Bridges program. This role reports to the Program Manager and is ideal for someone passionate about the well-being of transition-age youth and skilled at building relationships with community stakeholders. The Case Coordinator is responsible for the Warm Handoff of youth transitioning from foster care into the Bridges program, collaborating with Public Children Services Agency (PCSA) caseworkers, community stakeholders, and the Bridges team to ensure smooth transitions and provide essential support to young adults aging out of foster care. The goal is to foster partnerships and relationships that improve transition outcomes for youth, requiring a candidate who is passionate, curious, responsive, collaborative, and committed to innovation.

Requirements

A minimum of an Associate Degree in human services, criminal justice, or related field
Experience working in a social service or community-based organization
Familiarity with local community resources and networks

Responsibilities

Establish regular meeting cadence with the Regional Coordinator(s) and Intake and Referral Coordinator to build rapport, establish processes, and maintain consistent communication about transition-age youth.
Establish communication with the PCSA and Independent Living (IL) teams to develop rapport and relationships and provide education about their role during the Warm Handoff (WHO) process.
Partner with the PCSAs and Independent Living teams to develop processes, outline a communication plan, and institute a process to promote collaborative work in successfully supporting and transitioning youth from foster care to Bridges.
Serve as the primary contact prior to and during the initial stages of the WHO process, informing the PCSA and IL teams about the process and its expectations.

Gather information from young adults about their needs, strengths, goals, eligibility for the program, and barriers they are encountering.
Identify applicable resources to address the young adults' immediate needs, secure housing, and ensure they are actively working towards meeting Bridges' eligibility requirements.
Attend all meetings involving the young adult, which include but are not limited to court hearings, transition meetings, OhioRISE Child and Family team meetings, meetings involving county Boards of Developmental Disabilities, job development, residential treatment, guardianship, and/or probation.
Collaborate with various community stakeholders, including foster caregivers, CASA/GAL, probation officers, residential service providers, job developers, clinical therapists, mental health case managers, and housing resources in Galia, Muskingum, Lawrence, and surrounding counties.
Develop and implement Key Performance Indicators or KPIs for the WHO process, which includes data collection and analysis of touchpoints with the Hancock, Lucas, Wood, and other surrounding county youth emancipating from care, PCSAs, and stakeholders to evaluate and understand their impact on unique referrals and enrollments.
Design, conduct, and evaluate surveys to assess the experiences of young adults, PCSAs, and IL teams throughout the WHO process.
