About The Position

Pathway Family Services is seeking a full-time Case Coordinator to join their team. This role involves working with children and families who have experienced trauma, conflictual relationships, and are struggling in their daily lives. The Case Coordinator will manage a caseload of children in the Pathway Family Services program, overseeing all facets of their daily lives to determine case coordinating, treatment, and discharge plans. This role requires effective collaboration with therapists, youth care workers, school personnel, residents, and other stakeholders to improve the quality of treatment. The ideal candidate will possess personal characteristics that align with the agency's values of respect, integrity, and a commitment to ensuring a safe learning environment for residents. They should enjoy engaging with residents as they learn and grow, communicate effectively with various parties, and conduct themselves professionally as a positive role model.

Requirements

Bachelor’s degree in a human services field.
Must be at least 21 years of age.
Must have a valid driver’s license and meet agency underwriting standards if driving on agency business.
Must successfully complete the Safe Crisis Management training provided by the Agency.
Ability to communicate effectively with staff, residents, leadership, and members of the community.
Conduct oneself in a professional manner and be a positive role model.

Responsibilities
