About The Position

The EFFC Case Aide provides essential support and supervision for family interactions, including parent-child and sibling time, and offers transportation as needed. This role involves coaching families to foster healthy engagement, completing timely documentation, and collaborating with the case planning team on court documents and permanency reports. The Case Aide assists in gathering vital documents, accompanies families to various appointments (school, medical, treatment), and supports them through public assistance processes. They conduct home and community visits, intervene in crisis situations, and attend court appearances. The position requires active participation in interdisciplinary team meetings, fulfilling Mandated Reporter duties, and regular supervision meetings to discuss client cases. Effective communication and collaboration with internal SCO foster care staff, external agencies, and service providers are crucial. The Case Aide must adhere to SCO's personnel policies and perform other assigned duties.

Requirements

  • High School Diploma.
  • Approved background clearances and fingerprinting are required.
  • Excellent oral and written communication.
  • Basic computer literacy skills, including Microsoft Office.
  • Valid NYS Driver License required

Responsibilities

  • Support and supervise family time between parents and children, as well as siblings and other significant resources.
  • Provide transportation as needed.
  • Provide visit coaching between parents and children to support healthy family engagement.
  • Complete ongoing documentation in Connections with 7 business days of the event date.
  • Work in partnership with the case planning team on completion of Permanency Hearing Reports and court documents as it pertains to the Case Aide’s communication, contact, and collaboration with the family.
  • Support case planning staff in gathering or obtaining vital documents such as birth certificates, social security cards, etc.
  • Accompanying and supporting children and parents through various appointments including school, medical, and drug treatment, etc.
  • Support families through the Public Assistance process, PATH process, and provide referrals, as needed.
  • Meet with families in their home and/or community, as needed.
  • Intervene in crisis situations to help the child and family stabilize.
  • Attend and testify in court appearances, as needed.
  • Attend all interdisciplinary team meetings and actively provide information regarding services being provided to the child.
  • Fulfills duty of Mandated Reporter, which entails reporting any suspected instances of child abuse or maltreatment.
  • Meet with Supervisor one-on-one on a consistent basis to discuss safety and permanency issues of caseload.
  • Communicates and work effectively with SCO foster care support staff such as Case Planners, Socio-therapists, Fair Future Coaches and Education Specialists, Parent Advocates, and medical and mental health staff and with NYC Children’s Services, outside medical and mental health centers, schools, consultants, other SCO programs, and all other relevant parties.
  • Adheres to SCO’s personnel policies.
  • Performs other duties as assigned.
