About The Position

LSF is seeking a talented Case Aide who wants to make an impact in the lives of others. Purpose and Impact: The Case Aide is responsible for assisting in the day-to-day tasks to ensure that case management services are provided to clients in a timely manner. The Case Aide is required to maintain a flexible, organized, and efficient work schedule and is subject to work extended hours.

Requirements

  • High School Diploma or GED, or other relevant degree or equivalent experience.
  • Experience working with clients; comfortable working closely with diverse client base.
  • Clerical, typing and computer proficiency required.
  • Good written, verbal communication and interpersonal skills with persuasive abilities.
  • Must exercise discretion and good judgment in dealing with sensitive issues.
  • Able to work with the public.
  • Able to follow written and oral instructions.
  • Able to work independently and with minimal supervision (initiative).
  • Dependable and reliable.
  • Initiative and follow through to ensure all assignments are completed.
  • Bilingual is required (English/Spanish)
  • Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.
  • Must possess a clean/valid Florida Driver License, reliable transportation, and proof of Insurance (transcript of driving record will be requested from the Department of Motor Vehicle at the time of hire).
  • Able to sit and stand for long periods of time, utilizes telephone, computer, and other office equipment.
  • Able to work in a fast-paced environment.
  • Must be able to work flexible and long hours.

Responsibilities

  • Determines eligibility by requesting the official documents that proof that the participant is eligible for the program.
  • Must ensure that clients are properly enrolled into the program’s internal intake system.
  • Ensure DHS eligibility documents are uploaded to CMS and WEB-RSDS systems.
  • Completes ACCESS form for benefits such as Cash, Food Stamps, and Medicaid (if needed). Provide copy to client of Registration form and all documents sent to DCF.
  • Drive clients to facilitate program services, may include transportation to appointments, visits, interviews, or other appointments as per contractual requirements as needed.
  • Create client intake case files with necessary intake documents.
  • Assist with filing, closing, and with overall daily maintenance of records found in the physical case file.
  • Assist with compiling electronic and physical files for ORR or DCF upon request.
  • Assist case management with creating FSSP packets, data entry documentation and provide CRS orientation to client as part of CRS plan.
  • Ensure the provision of follow up service phone calls are conducted every 30 days of clients been registered in the program.
  • Assist with the coordination of referrals for in-house services.
  • Attend all organizational required trainings and facilitate trainings as needed.
  • Attend all departmental and program meetings to ensure that up-to-date information is received and/or provided and to ensure that changes to policy or practice are implemented and adhered to.
  • Assist with the submission of monthly reports.
  • Provide and facilitate information as needed for monitoring activities.
  • Travel as needed for trainings, conferences, or to transport clients.
  • Attend training that will enhance professional growth in the area of case management documentation, or other topics as deemed appropriate by the Program Manager.
  • Maintain a safe, clean, and hazard-free work area.
  • Ensure the proper supervision of youth at all times.
  • Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.
  • Able to react to change productively and handle other essential tasks as assigned.
  • Work on special projects with minimum supervision.
  • Back up in the absence of the legal assistant.
  • Performs other duties as assigned or required.
