About The Position

The Case Aide works closely with the Case Manager assisting with the case management tasks and facilitates the safe and timely release or discharge of children and youth, documents the provision of services in case file according to ORR requirements, state licensing standards, and Abbott House’s policies/procedures.

Requirements

  • HS diploma required
  • experience in child welfare/child protective services
  • Excellent writing and communication skills
  • proficiency both written and oral in Spanish and English
  • Candidate must have demonstrated ability to reach out and work with community resources and foster parents.
  • Driver’s License
  • Must be able to use a computer and able to learn software programs
  • Must be able to operate a fax, copier, scan, and other office equipment.
  • Must be able to speak, read and write fluently in the English language
  • Must be able to write in the English language using correct spelling, grammar, punctuation, etc.
  • Must have a valid Driver’s License and a clean driving record.

Responsibilities

  • Assist the Case Manager with case management duties, such as calling sponsors, collecting documents, filing documents, and uploading pertinent documents into the portal.
  • Responsible for collecting the recreation log, medication log, clothing log and weekly educational reports to present for staffing and to file in the hard copy file.
  • Assures the Discharge Package is complete, and child is given the discharge package and signs acknowledgement of receipt of discharge package.
  • Uploads the UC’s federal paperwork into the portal 24 hours upon arrival.
  • Provides the UC calls to sponsor, lawyer, and family.
  • Helps Case Manager to identify and screen prospective sponsors, URM programs and Post 18 plans.
  • Manages unification/ discharge process.
  • Concurrent planning to expedite safe and timely releases.
  • Effectively coordinates inter and intra-agency services to clients by working as a team with the providers (foster parents, clinicians, teachers, medical, legal, FFS, CFS, CC etc.).
  • Establishes effective collegial relationship with Lead Case Manager, Senior Case Manager and Lead Clinician.
  • Attends weekly staffing meetings and requests meetings with the FFS for complex cases.
  • Follows up on timely and appropriate referrals for medical services, health screening and mental health assessment of young people.
  • Participates in the 7day case management rotation.
  • Makes meaningful contributions to agency and department programming.
  • Carry out the job responsibilities in a professional manner.
  • Complete all required training.
  • Mandated Reporter.
  • Participates in individual, group, and department supervision.
  • May conduct home visits as requested to support case management team.
  • Attend and complete all mandatory ORR & Abbott House mandatory training on a timely basis.
  • Any other related duties as required.
