About The Position

Heritage Home Healthcare is seeking a compassionate and dependable PRN Caregiver to provide personalized in-home care for a client in the San Ysidro, NM (87053) area. This position offers approximately 15 hours per week with a flexible schedule based on the client's needs. In this role, you'll assist with daily living activities while promoting the client's comfort, safety, independence, and dignity. We're looking for someone who is reliable, observant, and passionate about providing high-quality, person-centered care.

Requirements

Must be at least 18 years of age.
Ability to work independently and follow directions accurately.
Strong interpersonal and communication skills.
Reliable transportation and willingness to travel to the client's home in the San Ysidro area.
Applicants should be comfortable providing personal care services in accordance with the client's care plan.

Nice To Haves

High school diploma or GED preferred, but not required.
Previous caregiving or home care experience preferred.

Responsibilities

Assist with personal care tasks including bathing, dressing, grooming, toileting, and eating.
Follow the client's individualized care plan, including assisting with exercises and delegated medication assistance, as appropriate.
Provide mobility assistance, including transfers to and from beds, chairs, or wheelchairs.
Perform light housekeeping to maintain a clean and safe home environment.
Provide companionship, emotional support, and encouragement.
Escort or transport the client to appointments or errands, if needed.
Observe and report changes in the client's condition, behavior, or needs to the clinical team.
Follow all care plans, policies, and delegation instructions provided by licensed healthcare professionals.
