Caregivers/Home Health Aides provide non-medical personal care and assistance as outlined in the client's care plan. Responsibilities may include assisting with bathing, grooming, dressing, and personal hygiene; helping with mobility, transfers, and ambulation; assisting with toileting and incontinence care; preparing and serving meals based on dietary guidelines; providing medication reminders; performing light housekeeping related to the client's care; assisting with laundry and linen changes; accompanying clients to appointments or errands, when assigned; providing companionship and emotional support; monitoring and reporting changes in the client's physical, mental, or emotional condition to the supervising nurse or agency; and recording care provided according to agency policies. Caregivers must follow infection control, safety, and confidentiality guidelines at all times.
