About The Position

The primary purpose of your job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves assisting residents with daily living activities, ensuring their comfort and safety, and maintaining accurate records. The position requires adherence to facility policies and procedures, effective communication with residents and staff, and a commitment to providing high-quality care.

Requirements

  • Must possess, as a minimum, a high school education or its equivalent.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Proficient in Microsoft Suite products.

Responsibilities

  • Provide routine daily care and services to assigned residents according to their assessment and care plan.
  • Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
  • Report all changes in residents’ condition to the Health and Wellness Director promptly.
  • Record all entries on notes, SPAs, and incident reports in an informative and descriptive manner.
  • Report all accidents and incidents observed to the Executive Director and Health and Wellness Director.
  • Maintain resident confidentiality and report suspected or known violations.
  • Report any known or suspected unauthorized attempt to access community’s information system.
  • Perform only trained care tasks.
  • Ensure resident rooms are ready and make beds.
  • Greet residents, escort them to rooms, and introduce them to others.
  • Assist residents with packing personal possessions for transfers or discharges.
  • Transport residents to new rooms or receiving areas.
  • Assist with loading/unloading residents to/from vehicles.
  • Create and maintain a warm, positive, and calm environment.
  • Meet with nursing personnel to identify and correct problem areas.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Answer resident calls promptly and check residents routinely.
  • Assist residents with identifying food arrangements and ensure water pitchers are clean and filled.
  • Perform after-meal care.
  • Receive the nursing report upon reporting for duty.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental and facility personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (dental and mouth care, bath, hair care, dressing/undressing).
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents in preparing for medical tests.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing residents for physical examinations.
  • Check residents routinely to ensure personal care needs are met.
  • Observe and report skin breakdowns to prevent pressure areas.
  • Report injuries of an unknown source, including skin tears.
  • Observe disoriented and comatose residents and record/report data.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse of any resident leaving/missing from the facility.
  • Use only trained equipment and report defective equipment.
  • Maintain confidentiality of all resident care information.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident's refusal of treatment requests and report them to a supervisor.
  • Give back rubs as instructed.
  • Assist residents with hair care functions.
  • Assist residents with nail care.
  • Shave male residents.
  • Keep hair on female residents clean as instructed.
  • Assist residents with bowel and bladder functions.
  • Maintain intake and output records.
  • Keep incontinent residents clean and dry.
  • Check and report bowel movements and character of stool.
  • Utilize protective and adaptive equipment as instructed.
  • Provide eye and ear care as instructed.
  • Weigh and measure residents as instructed.
  • Measure and record temperatures, pulse, and respirations (TPRs) as instructed.
  • Assist with the care of the end-of-life resident.
  • Assist with post-mortem care as instructed.
