About The Position

On Lok PACE is a non-profit organization dedicated to the care of elders through the Program of All-Inclusive Care for the Elderly (PACE). The PACE model focuses on serving seniors with chronic care needs and their families in their community. On Lok PACE addresses issues such as transportation, medication management, coordination of medical care, social interaction, and in-home support. Participants receive in-home care services and transportation to a PACE center for medical care, social activities, and other senior care services. The organization seeks individuals with expertise, drive, and passion to improve lives and advance healthcare, offering a rewarding career path.

Requirements

  • Minimum one-year experience working with frail or elderly required.
  • HHA or CNA required.
  • Understanding of PACE model gained through personal experience or research.
  • Must be flexible and have a positive attitude working across multiple settings.
  • Ability to work independently and as part of interdisciplinary team.
  • Must have current Basic First Aid and CPR certification.
  • Knows, uses and explains use of safety equipment for lifting; corrects others in a positive way "here let me help you and show you how this works".
  • Physical requirements: Ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and repeat motions of the wrist and hands. Must have visual acuity. Must be able to lift/move weight that is within individual physical ability and use appropriate lifting devices or request assistance when needed.
  • Basic English reading and writing required; bilingual ability may be required in specific assignments and daily participant interaction.
  • Basic computer skills; able to use or learn to use multiple applications such as email, electronic health record.

Nice To Haves

  • Minimum one-year experience working in home or clinic setting preferred.

Responsibilities

  • May perform duties of a Caregiver 1.
  • Provides onboarding and ongoing training for other Caregiver-1s and 2s using manuals and formal training materials as a guide.
  • Active participation in departmental meetings and family meetings as needed.
  • Other related duties as assigned.
