Requirements

• Must possess an active RN license.
• Proven hospice experience with a deep understanding of Medicare regulations and eligibility requirements for hospice care.
• Exceptional interpersonal, communication, and presentation skills.
• Robust organizational and time management skills are essential.
• Must be adaptable, patient-focused, and capable of handling challenging situations with tact and empathy.

Nice To Haves

• Hospital case managers / discharge planners
• ICU, ER, oncology, and cardiac nurses (high exposure to end-of-life care)
• Palliative care nurses
• Home health nurses ready for a more relationship-based role

Responsibilities

• Conduct hospice eligibility assessments based on thorough knowledge of Medicare Hospice Regulations.
• Educate patients, families, and facility staff about hospice services and patient-specific care plans.
• Act as a primary point of contact to ensure coordination and continuity of care.
• Utilize strong communication and time management skills to effectively manage multiple cases and priorities.
• Participate in interdisciplinary team meetings to review patient plans and compliance with hospice criteria.
• Provide ongoing support and education on the hospice process to healthcare providers within the community
