About The Position

The Care Transitions Manager - Social Worker is responsible for improving the health of those in the community. This role is part of a 255-bed, full-service hospital serving its community since 1987. The hospital is a Joint Commission-accredited Primary Stroke Center and Chest Pain Center and a Bariatric Surgery Center of Excellence and a Pathways to Excellence-designated hospital. It is the only choice in Denton for oncology services, women and infants’ services, and much more.

Requirements

• Master's Degree Social Work required upon hire.
• LMSW - Licensed Master Social Worker or LCSW - Licensed Clinical Social Worker required upon hire.
• CPR - Cardiopulmonary Resuscitation required upon hire.
• Knowledge of Microsoft Outlook and Office (Word, Excel).
• Customer service skills.
• Ability to engage in complex clinical decision-making.
• Strong oral and written communication skills.
• Strong commitment to interdisciplinary collaboration.
• Critical thinking, analysis and conflict resolution skills.
• Psychosocial and crisis intervention skills.
• Ability to prioritize and meet deadlines.

Nice To Haves

• Three (3) years of hospital/medical social work experience is preferred.
• One (1) year of discharge planning/care management experience is preferred.
• ACM - Accredited Case Manager is preferred upon hire.
• CCM - Certified Case Manager is preferred upon hire.
• Other ANCC is preferred.
• Working knowledge of medical necessity criteria preferred.
