About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Care Transitions Coordinator works to promote home care services, develop referral relationships, and coordinate patient admissions. The role includes on-site visits, maintaining referral and interaction records, meeting account and admission goals, and attending community events. The position travels 50% or more within the assigned territory and responds to customer complaints in a timely manner.

Requirements

Current RN or LPN license specific to the state(s) assigned; certain MSW, LCSW, PT, PTA, OT, OTA, SLP, and PA licenses may be considered in some cases.
At least 1 year of experience in a healthcare-related business development or outreach role.
Reliable transportation, a current driver's license, and liability insurance.

Responsibilities

Educate health care professionals, patients, and families about home care services.
Develop and maintain relationships with referral sources, including hospitals, nursing homes, and physician offices, to generate leads.
Coordinate patient admissions with the team, including obtaining required documentation for the referral-to-admission process.
Conduct on-site visits to assess patient needs and identify barriers to care.
Maintain accurate records of patient interactions and referrals.
Achieve admissions, product, and payor-mix goals.
Meet short- and long-term target account goals.
Document the minimum expected number of daily sales calls in CRM, including pre- and post-call notes.
Maintain complete, up-to-date CRM records of targeted referral sources in the territory.
Attend community events to promote Amedisys.
Respond to customer complaints in a timely manner.
Perform other duties as assigned.
