About The Position

Bring your passion to Texas Health so we are Better + Together. This role is responsible for coordinating the clerical and administrative processes for entity care transitions management. You will manage departmental mailbox, phone lines and/or fax. You will support transition planning activities as needed under the direction of care transition managers, including calling vendors, physician offices, clinics, etc., and faxing documents. You will schedule follow-up appointments and call insurance companies for referral approvals for patients. You will also assist/support Case Managers by going into patient rooms for signatures on documents and delivering Durable Medical Equipment (DME) like wheelchairs, walkers, and wound vacs to patients prior to discharge. Additionally, you will make copies, gather other data, collect and enter data, and submit for prior authorizations for transportation and post-acute disposition via the insurance portal. Other tasks and projects may be assigned.

Requirements

  • High School Diploma or Equivalent Required
  • 3 Years relevant experience Required
  • Knowledge of medical terminology preferred
  • Problem solving skills
  • Latitude for decision making and setting of priorities
  • Interpersonal skills and Communication skills
  • Ability to work with minimal supervision or without supervision when necessary
  • Proficient computer skills with experience in: MS Outlook, MS Office (Word, Excel), internet navigation, clinically based software, and financially based software
