The Montana Department of Public Health and Human Services (DPHHS), Behavioral Health and Developmental Disabilities Division (BHDD), Treatment Bureau is seeking a Care Transition Specialist. This role is part of the Care Transitions Section, which provides Critical Time Intervention (CTI) services to low-income Montanans transitioning from the Montana State Hospital to the community. CTI is a voluntary, evidence-based, person-centered program designed to assist individuals in connecting with necessary professional and personal supports to foster successful community reintegration and reduce future hospital admissions. This is a client-facing position that takes place in the community, not in an office setting. The specialist will typically work business hours with frequent in-state travel, requiring flexibility. Responsibilities include working with clients and providers to establish connections to care, as well as fostering personal relationships. The position reports directly to the Care Transitions Program Supervisor.
