About The Position

Care Transition Coordinators serve as the primary liaison between the company and referral sources. They are responsible for developing, maintaining, and servicing community relations in several settings. The role involves educating patients, caregivers, physicians, case managers, discharge planners, social workers, and others on the company’s programs and services. The Care Transition Coordinator is responsible for obtaining patient referrals within their assigned territory.

Requirements

  • An Associates Degree in Business, Nursing, Marketing, or Healthcare related field. In lieu of degree, five or more years of experience in Business to Business and/or industry selling experience.
  • One year of sales experience. One or more years of related nursing experience may be substituted for sales experience.

  • Work requires exerting force up to 50 pounds occasionally, and/or up to 25 pounds frequently, and/or up to 10 pounds constantly in moving objects or materials.

Nice To Haves

  • Bachelor’s Degree in Business, Nursing, Marketing or Healthcare Related Field.
  • Formal sales training also preferred.
  • Two to five years of sales experience.
  • One or more years of sales or nursing experience in home Health or Hospice.

Responsibilities

  • Manage Sales Performance: Consistently meet monthly Home Health and/or Hospice budgets. Review territory regularly with manager to optimize market potential. Participate in sales meetings to achieve company goals, being prepared, organized, and engaged.
  • Territory Management: Effectively and efficiently target assigned territory, communicating and educating on home health and hospice services. Develop and implement weekly and monthly call plans for optimal productivity. Engage in strategic planning for the territory. Identify new referral sources and maximize business growth opportunities. Efficiently cover the assigned territory on a weekly, monthly, and quarterly basis. Maintain knowledge of systems used to track referrals and referral sources to discuss trends at sales meetings. Meet territory-specific daily and weekly call quotas. Accurately maintain assignment reports.
  • Customer Knowledge and Market Dynamics: Understand referral sources' businesses, practices, organizational structures, and key decision-makers. Be aware of market conditions and trends, including opportunities and challenges. Assess market potential and differentiate between physician and facility opportunities. Understand the strengths and weaknesses of home health and hospice competitors, including their sales efforts and market share. Determine and maintain referral source potential and productivity.
  • Strong Selling Skills: Demonstrate appropriate pre-call planning and objectives. Utilize knowledge of the sales process, relationship building, and maintenance. Appropriately use resources such as sales materials, branch staff, medical directors, and marketing budgets. Possess the ability to problem-solve and resolve conflict.
  • Administrative Duties: Attend and be engaged in all meetings. Complete all assignments given by the manager on time. Document sales calls appropriately and by the deadline. Log expenses and complete reporting by the deadline. Follow up on all pending referrals. Complete timesheets and mileage reports by the deadline.
