About The Position

VHA Home HealthCare (VHA) is seeking a Care Team Supervisor (CTS) for a temporary, full-time contract position ending March 2028. The CTS is a fundamental role providing essential leadership and oversight to a dedicated geographical team of PSWs and Care Team Scheduling Coordinators, ensuring top-notch care and support to clients. This role responds to escalated client or PSW issues, which may involve joint visits with clients, families, and funders. The position combines nursing clinical expertise with operational/systems proficiency to achieve positive outcomes for clients, service providers, office staff, and funders.

Requirements

  • Registered RN or RPN with CNO registration in good standing.
  • Minimum of 3 years experience in community health preferred.
  • 2 to 3 years of supervisor experience in health care strongly preferred.
  • Knowledge of the Occupational Health and Safety Act, relevant government legislation including Employment Standards Act, the Ontario Human Rights Code, PHIPA.
  • Computer literacy (i.e., Power BI) and Microsoft Office applications skills required.
  • Familiarity with GoldCare client management program an asset.
  • Proven skill with data informed decision making and report analysis.
  • Valid driver’s license and use of a car on the job is a requirement.
  • Weekend and after-hours commitments are required.

Responsibilities

  • Leads a team of Personal Support Workers, Personal Support Coaches, and Care Team Scheduling Coordinators.
  • Accountable for all Performance Management activities including performance appraisals, coaching, mentoring, and developing staff.
  • Implements progressive disciplinary action as required.
  • Listens and addresses clients and families concerns and conveys understanding and commitment to explore solutions and options.
  • Monitors operational performance and key performance indicators to assist with operational decision making.
  • Supports QI projects and process improvements.
