This position provides ancillary health services and care for residents in the community, including participating in the development of and implementing the services plan for each resident and functioning as a role model and coach for nursing assistants and medication aides. The role requires providing general care and treatment for residents within the scope of licensure, conducting nursing evaluations/assessments, completing service plans with residents and/or families, ensuring infection control procedures are followed, assisting with move-in and orientation of new residents, administering medications and treatments, and observing/documenting results. The position also involves functioning as a liaison and advocate for residents with physicians and outside nursing staff, completing shift reports, providing shift supervision, coaching team members, and potentially completing team member evaluations. Additionally, the role serves as a point of contact for resident healthcare and services, proactively communicating changes in resident condition, meeting with families to discuss health care services, and promoting the scope of services to external medical providers.
