About The Position

The Care Team Specialist (CTS) at Crocker Residential plays a vital role in fostering a safe, supportive, and dignified environment for our residents in a home-like RCFE setting. Grounded in a harm reduction approach and a “do whatever it takes” mindset, the CTS serves as a compassionate, observant presence, monitoring shared spaces, upholding facility rules, and ensuring compliance with licensing standards. Through proactive relationship building, trauma-informed engagement, and non-violent conflict de-escalation, the CTS helps maintain a respectful and stable community. Responsibilities include maintaining accurate sign-in/out logs, conducting safety checks of rooms and belongings, leading emergency preparedness drills, and documenting incidents with care and precision. Crocker Residential Care is a licensed Residential Care Facility for the Elderly (RCFE) that provides 24/7 non-medical care and supervision to adults aged 60 and older who live with chronic health conditions. Our program offers a home-like environment with assistance in activities of daily living (ADLs), medication reminders, and a focus on social engagement and independence.

Requirements

  • High school diploma or equivalent
  • Experience working with individuals experiencing homelessness.
  • Strong communication skills, particularly in de-escalation and crisis intervention.
  • Ability to work with individuals actively using substances in a nonjudgmental, harm reduction-focused manner.
  • Ability to work in a non-traditional clinical setting (e.g., outdoor park-lets, drop-in spaces).
  • Demonstrated knowledge of harm reduction, trauma-informed care, and housing-first principles.
  • Proficiency in basic computer skills (email, data entry, Microsoft Word/Excel).
  • Ability to work flexible hours, including evenings and weekends.
  • Bilingual (English/Spanish) preferred.

Responsibilities

  • Establish a positive rapport with participants, providing a safe and supportive environment.
  • Apply harm reduction and trauma-informed care principles in all interactions.
  • Monitor common areas and building perimeter to ensure participant safety and security.
  • Provide crisis intervention and de-escalation support using nonviolent techniques.
  • Consistently monitor and enforce the facility’s House Rules and ensure ongoing compliance with all licensing regulations required for Crocker’s operation as a licensed RCFE.
  • Maintain sign-in/out logs for participants and visitors at the front desk.
  • Conduct belongings searches and room checks as needed to uphold safety protocols.
  • Serve as floor lead during fire drills, evacuations, and emergency situations.
  • Complete detailed reports on incidents, participant concerns, and program operations.
  • Assist with participant intake, engagement, and connection to supportive services.
  • Facilitate communication between participants and care team members.
  • Respond to participant concerns with empathy, de-escalation strategies, and problem-solving techniques.
  • Ensure program areas remain clean, organized, and welcoming.
  • Collaborate with emergency responders, behavioral health teams, and harm reduction specialists as needed.
  • Maintain strict confidentiality and adhere to all organizational policies.
  • Utilize AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) principles in all participant and team interactions.
  • Other duties as assigned.
