The Registered Nurse/Psychiatric Nurse practices within the context of an integrated, team-based primary care and population health model. They work within the Scope of Practice and Professional/Practice Standards of the British Columbia College of Nurses and Midwives (BCCNM) and VCH policies. The role involves collaborating to coordinate and develop a plan of care within a team-based, interdisciplinary model that includes Primary Care, Mental Health and Substance Use, Home Health, and other community partners. The nurse cares for clients and families facing health challenges such as chronic disease and mental health/substance use issues, often affected by social determinants of health, to establish and coordinate care plans and support transitions between services. They use health interventions and self-management principles to conduct physical and mental health assessments, identify problems, and address variances from the care plan in collaboration with the team and other services. The nurse also acts as a resource, providing nursing support based on theory and practice for clients with complex health needs. Direct care activities include assessing, planning, implementing, evaluating, and documenting client care within the clinic or on outreach/home-based visits, collaboratively with partners. Establishing and maintaining therapeutic relationships based on respect is key.
