About The Position

The Care Team Lead is responsible for leading care staff during their shift, fostering a positive and collaborative work environment focused on resident wellness. This role involves assisting with new team member orientation and training, communicating with families and residents, managing call-offs, and delegating daily care duties. Additionally, the Care Team Lead provides support for medication administration, including passing medications and maintaining med carts, to ensure quality care and team efficiency.

Requirements

  • High school diploma/GED.
  • Successful completion of Care Specialist I and II curriculum within 90 days of hire.
  • At least 6 months of recent experience as a Care Specialist or equivalent experience.
  • CPR/First Aid certification or obtain within thirty (30) days of hire.
  • Demonstrable knowledge and experience using Microsoft Office in a Windows environment.
  • Capacity to master company-specific software.
  • Ability to multi-task, prioritize, and work with minimal supervision.
  • Effective oral and written communication skills.
  • Professional telephone etiquette and customer service skills.
  • Superior communication and interpersonal skills.
  • Ability to communicate in a respectful and professional manner with residents, families, co-workers, and leadership staff.
  • Developed organizational skills.
  • Ability to problem-solve and demonstrate good judgment.

Responsibilities

  • Serve as the primary point of contact for team members during the shift.
  • Participate in the orientation and training of new care team members.
  • Foster a positive work environment and support the development of skills among care team members.
  • Ensure residents are treated with kindness and respect through mentoring and modeling appropriate care and customer service.
  • Protect residents from accidents and injuries by practicing safe work habits and safe lifting techniques.
  • Be responsible for resident safety in emergency situations.
  • Assist with schedule coverage needs as necessary.
  • Review and provide resident care as outlined in resident service plans.
  • Respond promptly to resident call signals, door alarms, and safety alarms.
  • Administer and pass medications to residents according to physician orders and established policies.
  • Review and process physician orders using the eMAR system with pharmacy support.
  • Conduct departmental reviews and audits, reporting findings to the Director of Resident Care.
  • Ensure adequate medication and supply inventory for residents.
  • Observe and report non-emergent resident behavior and condition changes to the Care Coordinator and/or Director of Resident Care.
  • Complete daily charting and documentation as required.
  • Follow and comply with infection control, safety, emergency, and confidentiality policies and procedures.
  • Promote positive relationships with residents, staff, and visitors.
  • Remain awake and alert during the shift.
  • Perform any other duties as requested within the scope of responsibility and job requirements.
