About The Position

The employee will develop effective relationship with the variety of individuals served, build rapport, and maintain appropriate boundaries. The employee will balance decision making between the needs of oneself, others, and the organization at the same time. Adheres to all Federal, State, and County regulations and achieves performance outcomes.

Requirements

  • Must be 18 years of age or older and have High School diploma or equivalency.
  • Must be able to read and write in the English language to complete their job duties successfully.
  • Must complete First Aid and CPR training upon hire and prior to entering the shelter.
  • Must demonstrate the ability to relate to individuals in a meaningful and empathetic way.
  • Must be willing able to work with persons who may display severe destructive behavior such as self-injurious behavior (SIB), aggression, property destruction, noncompliance, tantrums, elopement, and other challenging behaviors and be able to implement behavioral programs.
  • Must be willing to work as part of the Interdisciplinary Team.
  • Must have a valid driver’s license and a good driving record.
  • Two years of developmental disability or mental health experience in residential setting.

Responsibilities

  • Ensures the welfare, safety, health and rights of the individuals we serve.
  • Participates in the ISP process for the individuals we serve as requested.
  • Ability to follow a prepared schedule and routine for each individual we serve.
  • Ability to work flexible hours to accommodate staffing needs via On-Shift. Ability to deploy to other departments to fill staff vacancies as directed by supervisor.
  • Operates and maintains property and equipment safely and in good repair.
  • Supervise, assists and teaches individual personal care skills as directed per ISP. Documents as required.
  • Assists individuals to prepare and serve three nutritious meals daily. Follows the prepared menus, where required.
  • Engages the individuals with daily chores, activities, and programs at all times.
  • Observes individuals’ activities, documents such and reports unusual behavior and/or occurrences to the appropriate Manager. Help individuals work through conflict with others living in the Adult Interim Care Home.
  • Models age-appropriate and socially accepted behavior. Dresses according to the activities of the day or as deemed appropriate by the Manager.
  • Displays an investment in the concept of normalization and an understanding of the agency’s mission.
  • Transports the individuals we serve to and from community activities for the purpose of recreation, leisure and medical appointments as well as keeping individuals informed of activities of interest.
  • Maintains and dispenses required medications and treatments, internal and external, with utmost care and accuracy as directed by physician’s orders and in accordance with Medicaid and Waiver regulations.
  • Attends on-going scheduled training sessions, in-service programs and designated staff meetings, and ISP meetings if requested by members of the team (CPI, Positive and Person-Centered Culture & Delegated Nursing Cat I, II, and III completed within the first six months). Will complete DoDD online training by the time they reach the equivalent of 2 years full time experience.
  • Ensures confidentiality of written and verbal information pertaining to the individual and his/her family.
  • Maintains accurate and complete records of individuals’ personal possessions.
  • Assists individuals as needed with financial matters; ensure receipts are collected and submitted timely.
  • Documents all services provided to the individuals we serve timely and accurately. Communicate observations and interactions with individuals to Adult Interim Care Home management.
  • Is accountable for reporting and documenting, MUI’s and UI’s in a timely manner.
  • Ensures home is clean and orderly with regard to premises, equipment and furnishings. Tasks include, but are not limited to windows, floors, toilets, and walls.
  • Maintains interior and exterior of agency vehicles in a clean and orderly manner. Operates vehicles in a safe manner.
  • Notifies Management timely for emergencies, equipment, home and major vehicle repairs. Ability to handle communications with other providers, law enforcement and Montgomery County Developmental Disability.
  • Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
  • Adhere to all policies and procedures of the organization regarding the use of technology and technology-based information.
  • Assists Home Manager in purchasing supplies for the house and the individuals, including groceries etc.
  • Monitors quantity of medications available for needs of the individuals we serve and orders refills as needed.
  • Maintains vehicle upkeep (6 month tune-up, monthly oil checks & tire rotations, etc.). Ensures regularly scheduled preventative maintenance tasks are complete. Operates vehicles in a safe manner.
  • On scheduled days, assists Home Manager by being on call for scheduled length of time.
  • Oversee intakes as scheduled- this includes assisting individuals and families with intake packets, leases, completing inventories, and going through medications.
  • Schedule and transport residents to medical appointments as directed, ensuring all documentation is provided from each medical appointment.
  • Train new employees as directed. Mentor Care Specialists in the home as needed and as directed. Proactively solves issues and resolves conflicts not requiring immediate management attention. Report concerns to the Home Manager timely.
  • Dispose of non controlled extra medications kept in the manager’s office monthly. Both Team Leads will work together on this task.
  • Assist with completion of weekly and monthly paperwork as directed.
  • Performs other duties as directed.
