About The Position

Established in 1948, The Mission of ASCEND Mental Wellness is to encourage and assist individuals in discovering pathways to their optimal mental health and wellness, while supporting hope and awareness of well-being in our community. We are seeking a compassionate and organized Care Plan Specialist to join our Restorative Residential Services team at our Community Living Apartments & Satellite Apartments. In this role, you'll work collaboratively with individuals, families, and multidisciplinary providers to develop person-centered service plans, coordinate care, and support individuals as they build independent living skills and work toward their goals. Community Residences: Community Living Apartments The Community Living Apartments Program offers two types of living arrangements: Maple Street and Satellite apartments. Both include support and guidance through individualized service plans created in partnership with the service recipient. The first step in treatment is defining and developing a rehabilitation plan which aids a person's movement to the next stage in their recovery. This individualized, highly detailed plan clarifies needs, charts a course of action and provides the framework to evaluate progress. Services available to residents include crisis intervention, rehabilitation counseling, social and living skills development (money management, personal hygiene, food preparation, meal planning, recreation activities, etc.), symptom and medication management, community integration and resource development. On duty around the clock staff work closely with each resident to maximize stability and help in the transition to a less restrictive environment. Longer term assistance is available for people with problems preventing them from living outside a supportive setting.

Requirements

  • A Master's or Bachelor's in Human Services or
  • An Associate's degree in a Human Services-related field with three years' experience in Human Services or
  • A high school degree with five years' experience in Human Services
  • Valid NYS driver's license acceptable to Agency insurance standards.
  • Should be compassionate and demonstrate a commitment to helping people with mental illness and/or co-occurring disorders.
  • Must have the ability to work independently, have good problem solving skills and have the ability to work as part of a multi-disciplinary team
  • Adequate computer skills

Responsibilities

  • Participate in implementing program policies, referral and discharge procedures, follow up procedures, etc.
  • Develop individual service plans with program residents and train and support Residential Specialist on the implementation of resident's goals.
  • Coordinate with other care providers within the agency and outside of the agency (i.e. - day program, medical and mental health providers).
  • Coordinate and facilitate Treatment Team Meetings.
  • Work with the resident's families as appropriate.
  • Help establish a pleasant, functional atmosphere within the residence, provide unobtrusive leadership as needed for house meetings, support, advice, and supervise resident's daily living activities (housework, menus, shopping, hygiene, medication, budgeting, etc.),develop and implement specific programs in living skills for residents as indicated.
  • Encourage effective communication, personal and interpersonal awareness and independence among staff and residents.
  • Assist in the instruction to residents in the use of house equipment, safety procedures and routines.
  • Provide informal case management services and counseling to residents as needed.
  • Maintain adequate records/files.
  • Attend regularly scheduled staff meetings and required in-service trainings.
  • Act as On Call as scheduled.
  • Facilitate discharge/transition of residents and provide follow up services to them.
  • Represent the agency in a positive manner.
  • Assist in OMH certification/re-certification processes as needed.
  • Complete special projects as assigned by supervisor.
  • Maintain a safe environment for all clients and staff.
  • Maintain effective communication with other staff members and supervisor.
  • Preserve human dignity and confidentiality of all clients and act in the best interest of those served through the Agency's programs and services.
  • Assure quality of work at all times.
  • Adhere to all laws and regulations applicable to the Agency's business.
  • Adhere to the Agency's Corporate Compliance Program and established policies and procedures.
  • Positively represent the Agency within the community at all times.
  • Have adequate computer skills, or more refined for specific job titles.
  • Actively participate in scheduled team/staff meetings as scheduled by supervisor or as indicated by the program's services.
  • Actively participate in Agency sponsored staff development and in-service educational programs as deemed necessary by supervisor, including mandatory trainings assigned per job title.
