About The Position

This is a Per-Diem position for Care Partners, also known as Certified Nursing Assistants (CNA), Home Health Aides (HHA), or Personal Care Assistants (PCA). The role involves assisting residents with their daily living activities (ADLs), ensuring their safety and well-being, and supporting the care team in providing high-quality care. Flexibility with scheduling and assignments is essential, as employees may be required to work outside their regularly scheduled hours with little to no advance notice. The position requires maintaining certifications and collaborating with peers.

Requirements

  • Certified Nursing Assistant (CNA), Home Health Aide (HHA) or Personal Care Assistant (PCA).
  • CPR/First Aid certification.
  • Knowledge of, and/or, 1 yr. of experience working with people with Alzheimer’s Disease and/or related disorders.
  • Sensitivity to issues of aging.
  • Ability to be flexible with scheduling and work assignments.
  • Ability to work as a team.
  • Maintain all certifications as required for employment.

Responsibilities

  • Assist residents with ADL’s, including toileting, bathing, dressing, and feeding.
  • Assist residents with ambulation and mobility devices (canes, walkers, wheelchairs).
  • Help residents with ADL devices like glasses and hearing aids.
  • Perform treatments such as vital signs, weights, temperatures, skin care, and range of motion exercises.
  • Assist residents with medications under the supervision of the Wellness Director.
  • Cue residents and provide reminders for medication.
  • Document information in and refer to record-keeping systems (diabetic list, toileting book, dietary needs book, daily communication book, flow sheets).
  • Assist the multi-disciplinary team in developing, implementing, and evaluating resident care plans.
  • Attend and contribute to care planning meetings.
  • Escort residents to medical appointments within the community.
  • Maintain cleanliness in resident rooms, bathrooms, storage areas, and common areas.
  • Ensure necessary supplies for resident care are in stock and communicate needs to the Resident Care Director.
  • Observe and report unsanitary conditions, fire, or safety hazards.
  • Inform the Clinical Care Coordinator of faulty equipment.
  • Observe and report any changes in resident status (physical, cognitive, emotional) to the Wellness Director and/or Clinical Care Coordinator.
  • Support the Therapeutic Recreation Department by assisting in daily activities and special events.
  • Assist residents to and from recreation programming.
  • Work collaboratively with peers as a team.
  • Maintain all required certifications.
  • Assist the Resident Care Director and Clinical Care Coordinator in training new staff.
  • Responsible for residents' safety and well-being during night shifts (11 PM-7 AM).
  • Provide ad lib recreation programming for awake residents during night shifts.
  • Maintain control of the hand-held security radio during night shifts.
  • Document resident incidents/issues for communication to the day shift during night shifts.
