About The Position

This is a Per-Diem position for Care Partners, also known as Certified Nursing Assistants (CNA), Home Health Aides (HHA), or Personal Care Assistants (PCA). The role involves assisting residents with daily living activities (ADLs), performing treatments, assisting with medications under supervision, documenting resident information, participating in care plan development, maintaining cleanliness, observing and reporting safety hazards and changes in resident status, supporting the Therapeutic Recreation Department, and working collaboratively with a team. Flexibility with scheduling and work assignments is essential, as employees may be required to work outside regularly scheduled hours with or without advanced notice. The night shift (11 PM-7 AM) has specific responsibilities including ensuring resident safety, providing recreation programming for awake residents, managing a security radio, and documenting incidents for the day shift.

Requirements

  • Certified Nursing Assistant (CNA), Home Health Aide (HHA) or Personal Care Assistant (PCA).
  • CPR/First Aid certification.
  • Knowledge of, and/or, 1 yr. of experience working with people with Alzheimer’s Disease and/or related disorders.
  • Sensitivity to issues of aging.
  • Ability to be flexible with scheduling and work assignments.
  • Ability to work as a team.
  • Maintain all certifications as required for employment.

Responsibilities

  • Assist residents with ADL’s, including toileting, bathing, dressing and feeding.
  • Assist residents with ambulation and mobility devices (canes, walkers, wheelchairs).
  • Help residents with additional ADL devices, such as glasses and hearing aides.
  • Perform treatments, including vital signs, weights, temperatures, skin care, and range of motion exercises.
  • Help residents with medications under the supervision of the Wellness Director, including cueing and reminders.
  • Document in and refer to record-keeping systems (diabetic list, toileting book, dietary needs book, daily communication book, flow sheets).
  • Assist the multi-disciplinary team in the development, implementation, and evaluation of resident care plans.
  • Attend and contribute to care planning meetings as assigned.
  • Escort residents to medical appointments within the community.
  • Maintain cleanliness in resident’s rooms, bathrooms, storage areas, and common areas.
  • Ensure needed supplies for resident care are in stock and communicate needs to the Resident Care Director.
  • Observe and report unsanitary conditions, fire or safety hazards.
  • Inform the Clinical Care Coordinator of any faulty equipment needing replacement.
  • Observe and immediately report any changes in resident’s physical or cognitive/emotional status to the Wellness Director and/or Clinical Care Coordinator.
  • Support the Therapeutic Recreation Department by assisting in daily activities and special events.
  • Assist residents to and from recreation programming.
  • Work as assigned and be flexible with hours; may be required to work outside regularly scheduled hours/programs with or without advanced notice.
  • Work collaboratively with peers as a team.
  • Maintain all certifications required for employment.
  • Assist Resident Care Director and Clinical Care Coordinator in training new staff.
  • Ensure resident safety and well-being during night shifts (11 PM-7 AM).
  • Provide ad lib recreation programming for awake residents during night shifts.
  • Maintain control of hand-held security radio during night shifts.
  • Document resident incidents/issues for communication to the day shift during night shifts.
