CareLinx is a healthcare technology platform that connects families with non-medical and in-home caregivers. The Care Partner role is crucial in ensuring that caregivers have a smooth, engaging, and exciting experience at CareLinx. In this role, you will serve as a point of contact for caregivers throughout their CareLinx journey after onboarding. You will build trusted relationships with caregivers, helping them identify opportunities that align with their skills, availability, and preferences while matching them with members who need care. Your responsibilities include coordinating caregiver and member matches, supporting caregivers through scheduling and placement, resolving questions and barriers that may impact successful care delivery, and helping ensure continuity of care. You will maintain ongoing communication with caregivers, providing coaching, resources, and guidance that enable them to succeed, remain engaged, and deliver exceptional care. Through proactive support and relationship management, you will help caregivers maximize their success on the CareLinx platform while contributing to positive outcomes for both caregivers and members.
