Language Differential Pay Available! Terros Health is hiring a Full-Time Care Navigator for our Mitchell location in Phoenix, AZ. High School Diploma or GED in Combination With One (1) Year of Behavioral Health Experience. Location: 40 E Mitchell Dr # 100, Phoenix, AZ 85012 Schedule: Full-time Monday - Friday 8:00am - 5:00pm Full Benefits, Including 401K and Generous PTO/PST (4+ weeks) Position Summary: The Care Navigator will work to provide support and community engagement for the Turning the Tide II (T3-II) program. T3-II is a program catered towards individuals in the community who live with a substance use disorder (SUD) and/ or a COD (Co-Occurring Disorder) or are living with or at risk of acquiring HIV. The Care Navigator assists the Program Supervisor to meet the deliverables of the project’s intervention to improve health outcomes of individuals throughout Maricopa County, as described contractually with the Substance Abuse Mental Health Services Administration (SAMHSA) and all other entities with which Terros Health has contracted.This position reports to the Program Supervisor.

Requirements

  • High School Diploma or GED in combination with life/work experience working in the field of HIV/STI prevention, public health and or substance use/mental health field.
  • Experience in health education and working in an HIV prevention capacity.
  • High level of competency in Microsoft Word, Intermediate Level Excel, PowerPoint, Outlook.
  • Flexibility and ability to work evenings and weekends if the Program Supervisor deems it necessary to achieve the objectives of the program.
  • Willing to work with all cultures and sub-cultures that comprise the population of Maricopa County in a non-judgmental and value clear manner.
  • Participate in additional training to meet the needs of prevention program(s) including phlebotomy training under the direction of the department management team.
  • Must be able to be certified in CPR/First Aid within sixty (60) days.
  • Willingness to undergo phlebotomy certification is required.
  • Must pass a TB test.
  • Must pass a criminal background check.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy.

Nice To Haves

  • Bilingual in English and Spanish preferred.
  • Phlebotomy trained and certified preferred but not required.

Responsibilities

  • Educate, coach, and empower clients/patients living with HIV or at risk of HIV to obtain and maintain medical stability.
  • Accompany clients/patients to routine primary care appointments and to other health care and social services encounters, as warranted.
  • Deliver HIV health promotion curriculum and track clients/patients progress through the curriculum.
  • Administer CLIA-waived rapid HIV and Hepatitis C screenings in addition to screenings for sexually transmitted infections (STI) through venipuncture blood draws and/or urine collection methods.
  • Meet regularly, in person with clients/patients on time per individual track level.
  • Coordinate logistical support, appointment reminders, transportation and other mechanisms for support to overcome barriers.
  • Collaborate with the patient, care coordinator and primary care provider to develop and follow up on the integration of identified services through the intake assessment and service plan.
  • Provide a high degree of collaboration and communication so that clients/patients are connected to integrated health services programs.
