About The Position

Be a Steady Guide in the Recovery Journey Recovery isn’t linear—and no one should navigate it alone. As a Care Navigator in our residential Substance Use Disorder (SUD) treatment program, you’ll be a consistent, trusted presence for clients as they move through treatment, transition between levels of care, and prepare for life beyond our doors. You’ll bridge clinical care, case management, and human connection—helping clients feel supported, informed, and empowered during some of the most vulnerable moments of their lives.

Requirements

  • Experience in substance use disorder treatment, behavioral health, case management, or care coordination
  • Strong interpersonal and communication skills with a client-centered approach
  • Ability to navigate complex systems while maintaining empathy and professionalism
  • CPI, PMAB, or equivalent de-escalation training (or willingness to obtain upon hire)
  • Reliability and flexibility to work 12-hour day shifts
  • High school diploma or equivalent required

Nice To Haves

  • Bachelor’s degree in human services, psychology, social work, or related field preferred
  • Experience in recovery services or lived experience strongly valued.

Responsibilities

  • Serve as a primary point of contact for clients throughout their residential stay
  • Support admissions, orientation, and transitions between levels of care
  • Coordinate care with nursing, providers, therapists, case managers, and peer support staff
  • Assist clients with treatment navigation, scheduling, and understanding next steps
  • Provide therapeutic support using motivational interviewing, trauma-informed communication, and de-escalation techniques
  • Monitor client engagement, identify barriers to treatment, and escalate concerns appropriately
  • Support crisis situations with grounding techniques and collaboration with clinical staff
  • Assist with discharge planning, aftercare coordination, and warm handoffs to outpatient or community providers
  • Maintain timely, accurate documentation in the EMR
  • Help promote a safe, structured, recovery-oriented milieu during day shift hours
