About The Position
Unison Health is seeking a full-time Care Navigator to support clients at the Cherry Street Ministry Mission. This position manages an assigned caseload and assists clients with securing and maintaining stable housing, accessing community resources, and overcoming barriers to independence. The Care Navigator will provide case management services, monitor housing stability, maintain client confidentiality, and participate in team meetings, supervision, and required trainings.
Requirements
- High school diploma required.
- Minimum of a CDCA license required.
- Must be able to work a flexible schedule that may occasionally include evenings and weekends.
Nice To Haves
- Four-year degree in social work, psychology, or related field preferred.
- LCDC II/III or LICDC preferred.
Responsibilities
- Assist clients (assigned caseload) to secure housing within the community and monitor their placement to ensure stability of housing.
- Perform case management duties, assisting clients with overcoming barriers of obtaining and maintaining resources.
- Maintain confidentiality and ensure release of information is handled in accordance with federal guidelines and agency policy/procedures.
- Participate in team meetings, supervision, in-service training, and conferences according to established policies, procedures, and applicable regulations.
