Marin Community Clinics, founded in 1972, is a multi-clinic network providing integrated primary care, dental, behavioral, specialty, and referral services. As a Federally Qualified Health Center (FQHC), they serve nearly 40,000 individuals annually in Marin County and have received national awards from HRSA. Their mission is to promote health and wellness through excellent, compassionate care for all. The Medical Assistant (MA) or non-Medical Assistant Care Navigator is a core member of an integrated care team, including primary care providers and behavioral health providers. Reporting to the Clinic Directors, this role coordinates and supports behavioral health and medical services with administrative tasks. The Care Navigator addresses social determinants of health by screening patients, connecting them to resources for transportation, housing, food insecurity, domestic violence, and more, thereby increasing healthcare accessibility.
