About The Position

River Vista Behavioral Health is a new, 128-bed facility providing advanced behavioral health services. The hospital employs over 250 people. We are seeking a Step-Down Outpatient Coordinator – (PHP/IOP) to join our Outpatient Clinic Team. This role is responsible for coordinating Step-Down admission/registration for the Intensive Outpatient Program (IOP) and providing case management services to the Outpatient program. The position involves significant community contact and collaboration with utilization review, business office, and clinical staff to ensure communication continuity. The role meets with eligible In-Patients to discuss PHP/IOP services as part of their continuum of care and ensures proper referrals are made. Registration functions include data entry, handling inquiry calls, and scheduling evaluations and admissions.

Requirements

  • Bachelor’s degree from an accredited college or university in social work, psychology, mental health, or a related field.
  • A minimum of two (2) years of direct clinical experience in a psychiatric or mental health setting.
  • Thorough knowledge of The Joint Commission, OSHA, and state and federal regulatory requirements.
  • CPR certification.
  • Successful completion of Crisis Prevention Intervention (CPI) training (may be obtained during new hire orientation).
  • Ability to react calmly and effectively in emergency situations.
  • Knowledge and utilization of crisis intervention techniques.
  • Excellent customer relations skills.
  • Excellent organizational and interpersonal communication skills.
  • Skill in interacting with multiple individuals with diversified roles and perspectives.

  • A strong knowledge of The Joint Commission, OSHA regulations, patient rights standards, and all other applicable federal and state laws and regulations governing mental health care facilities.

Responsibilities

  • Coordinate Step-Down admissions/registrations for IOP and provide case management services to the Outpatient program.
  • Meet with eligible In-Patient individuals to discuss the benefits of PHP/IOP services as part of their continuum of care treatment and ensure the proper referral is made.
  • Handle inquiry calls and schedule evaluations and admissions.
  • Meet with every eligible patient to discuss PHP/IOP services, methods of transportation, virtual programming options, and potential start-dates.
  • Telephone screen all inquiry calls, make appropriate referrals for emergency situations, and schedule appointments.
  • Document inquiry calls in the system database.
  • Collect data-points specific to the step-down process, including Eligibility, Referrals, Refusals, Reason for Refusal, and Attended, and enter this data into Midas.
  • Coordinate with other mental health facilities to obtain clinical referral information and schedule referrals within a timely manner (3 business days).
  • Post the appointment schedule for the following day and distribute it to appropriate River Vista staff.
  • Make follow-up calls to patients who fail to show for scheduled appointments.
  • Assist with insurance benefits verification and completion of financial responsibility agreements.
  • Complete Admission paperwork in accordance with policy.
  • Serve as a source of information for referral sources regarding facility services.
  • Work closely with departments involved in eligibility verification (Social Services, Business Office, Utilization Review, Outpatient Departments) to confirm patient transition to PHP/IOP programming after inpatient discharge.
  • Collaborate with the Business Development Coordinator to provide referral source feedback and maintain the referral source database.
  • Collaborate with the Director of Transportation to ensure timely addition of patients to transportation routes and facilitate parents' transportation needs.
  • Ensure patients and families receive an orientation to the program.
  • Coordinate admissions with clinical staff to ensure timely admission to the program.
  • Coordinate transfer referrals from River Vista Behavioral Health, ensuring clinical data is received timely.
  • Provide assistance with the receptionist area as assigned.
  • Identify trends from collected data and report on opportunities to increase step-down referrals weekly.
  • Maintain knowledge of the Adolescent Program rules, protocols, and level system.
  • Maintain knowledge of adolescent stages of development and seek guidance from the nurse in charge when questions arise.
  • Recognize acting out behavior of the adolescent specific age group and set appropriate limits.
  • Relate to adolescent patients in an age-appropriate manner.
  • Utilize behavioral management techniques with an understanding of modifications for smaller body sizes, abandonment issues, and history of abuse.
  • Maintain knowledge of the Dual Diagnosis Program, program rules, and protocols.
  • Recognize acting out behavior of the Dual diagnosis specific group and set appropriate limits.
  • Relate to Dual Diagnosis patients in an age-appropriate manner.
  • Utilize behavioral management techniques with an understanding of modifications for smaller body sizes, abandonment issues, and history of abuse.
  • Use time constructively and in an organized manner to accomplish duties/responsibilities.
  • Participate in business development activities.
  • Comply with all facility, department, and corporate policies and procedures.
  • Attend all mandatory facility in-services and staff development activities.
  • Adhere to facility standards concerning conduct, dress, attendance, and punctuality.
