About The Position

Headway is building a Care Navigation & Triage program to help patients leaving emergency departments, inpatient psychiatric units, and partial hospitalization programs connect with appropriate mental health care quickly and safely. As a Care Navigator, you coordinate each patient's path from hospital referral to a confirmed first appointment. You serve as the main point of contact for hospital partners and patients, work with a licensed Clinical Assessor, and contribute to shaping a developing program. This is a part-time, remote contract role requiring a minimum commitment of 20 hours per week. Compensation is $43.50 per hour. The Care Navigator must establish or already have an LLC or S-Corp.

Requirements

  • At least 2 years of experience in behavioral health care coordination, case management, patient navigation, or hospital discharge planning.
  • Comfort working with recently discharged patients who may be in distress and ability to recognize when to involve clinical support.
  • Strong organization and ability to manage a high volume of referrals and follow-ups with tight turnaround while maintaining attention to detail.
  • Clear, warm communication with patients, hospital staff, and providers.
  • Persistence and resourcefulness in addressing practical barriers to care, such as insurance, transportation, and access to community resources.
  • Comfort building a new program and adapting to workflows that change as the program grows.
  • Must set up or already have an LLC or S-Corp.

Nice To Haves

  • Familiarity with insurance and community resources in Missouri or Virginia.

Responsibilities

  • Receive referrals from partner hospitals and serve as the main contact for discharge planners and care teams.
  • Route referrals according to established guidelines and a triage protocol, either directly to a Headway provider or to a Clinical Assessor for evaluation.
  • Schedule patients with the Clinical Assessor, then book them into the level of care recommended by the assessor, such as a Headway provider, specialty program, or community resource.
  • Follow up to confirm patients attended care and respond quickly if an appointment falls through.
  • Connect patients with practical support, including transportation, food, insurance and benefits questions, and coordinate between their providers.
  • Build and maintain a list of local referral resources in each market.
  • Follow escalation protocols and promptly involve clinical staff when a patient may be in crisis; level-of-care decisions are made only by licensed clinicians.
  • Document patient interactions and complete work in Headway's designated systems, following HIPAA and Headway privacy and confidentiality requirements. Patient information must not be stored or kept outside those systems.
