Reporting to the Manager of Integrated Client Services, the Care Navigator is responsible for promoting a stigma-free youth culture for mental illness, conducting detailed Community Mental Health Assessments, and assisting adolescents and young adults in prioritizing needs and developing short-term plans. The role also involves promoting opportunities for increased independence, championing community peer development, acting as a resource or advocate for accessing needed services and supports, and supporting reintegration into school, work, and social activities.

Requirements

  • Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; or a Registered Nurse or Registered Practical Nurse with current registration with the CNO; or Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F.
  • 1-2 years’ experience in a health related organization
  • Experience working with Adolescents/young adults with mental health issues
  • Personal lived experience in recovery from a mental health and/or addiction challenges.
  • A valid Ontario Driver’s License and access to a vehicle are required.

Responsibilities

  • Promotes a stigma free youth culture for mental illness.
  • Conducting and completing the detailed Community Mental Health Assessment.
  • Assisting A/YA in prioritizing needs/areas of consideration and to develop short-term plans to address these priorities.
  • Promotes opportunities for increased independence and champions community peer development.
  • Acting as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed needed.
  • Supporting reintegration into school, work and social activities.
