About The Position

This role requires a BSN from an accredited school of nursing (CCNE, ACEN, or CNEA). A Master's degree in a healthcare-related field or an MSN is preferred. The position also requires a current Indiana state RN license, 3-5 years of nursing experience, and 3-5 years of care management experience. Preferred care management settings include Home Health, Ambulatory Care, Community Public Health, Case Management, and coordinating care across multiple settings and providers. The individual must be able to complete job duties autonomously, possess demonstrated leadership and strong organizational skills, and have the ability to interpret and analyze information. Exceptional communication skills, both written and oral, are essential for positively influencing others, including physicians, healthcare providers, families, and patients. A strong work ethic built on proactivity and teamwork, along with the ability to navigate ambiguity using structured problem-solving techniques, is also required.

Requirements

  • BSN from accredited school of nursing (CCNE, ACEN, or CNEA).
