Care Manager - Internal Medicine
About The Position
The Care Manager coordinates the plan of care in collaboration with the provider, interdisciplinary team, patient, and family. The role facilitates care planning to enhance quality of patient care, optimize clinical outcomes and patient satisfaction, and manage the cost of care. The Care Manager also helps drive change by identifying opportunities for performance improvement, including day-to-day workflow, education, processes, and patient satisfaction. The Care Manager is accountable for a designated patient caseload and plans effectively to meet patient needs, manage length of service, promote efficient use of resources, and ensure each patient has a well-organized and safe plan of care. This position focuses on conducting annual wellness visits at ECU Health Internal Medicine, an outpatient clinic in Washington, North Carolina.
Requirements
- Associate Degree in Nursing or higher.
- Current Registered Nurse licensure from the North Carolina Board of Nursing.
- 0–3 years of Medical Case Management experience.
