About The Position

The Care Manager is a non-exempt, hourly PRN position that assists residents with daily living needs as indicated in their care plans. Duties may include personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. The role reports to the Clinical Services Director.

The position pays $20.00–$22.00 per hour. The associate helps maintain residents’ safety, comfort, dignity, and confidentiality and documents services provided during each shift.

Requirements

• Certified Nursing Assistant and/or DHS certification.
• High school diploma.
• Ability to perform the listed physical activities, including lifting or carrying up to 50 pounds and intermittent lifting and support of residents.
• Use proper body mechanics.

Nice To Haves

• Knowledge and experience working with the elderly population.
• Dementia experience is preferred.

Responsibilities

• Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in care plans.
• Maintain residents’ safety, comfort, dignity, and confidentiality.
• Escort residents to meals and activities.
• Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
• Document all services provided to assigned residents during the shift.
• Immediately report any change in a resident’s condition to a supervisor.
• Assist residents with daily needs, including food service, housekeeping, laundry, behavior management, socialization, and activities, as indicated in their care plans.
