About The Position

OCES is growing and expanding its services to 11 more towns covering the South Shore area. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. The organization's mission is to promote healthy, safe living for consumers, enabling them to stay active and engaged in their lives and communities. The Home Care (Care Manager) will work in the Home Care department to help, evaluate, initiate, and monitor home care services for consumers.

Requirements

Minimum of one year of experience in the human services field
Minimum of one year experience in the human services field in lieu of bachelor's degree with the ability to obtain a waiver from Executive Office of Aging Independence (AGE)
Excellent written and verbal skills
Excellent computer skills
Proficient with Microsoft Office Suite or related software

Responsibilities

Conduct comprehensive needs assessments and develop individualized care plans
Develop rapport with consumers to facilitate coordination, monitoring, and adjusting care plans as needed
Provide regular contact with consumers, including home visits, to ensure progress toward established goals
Contact and maintain communication with appropriate community resources to better coordinate delivery of consumer services
Maintain a detailed knowledge of current OCES policies, and standards, funding source regulations
Maintain accurate documentation
