About The Position

The Care Manager serves as the primary point of contact and advocate for families after a student’s initial enrollment period. This role is central to care coordination, communication, and collaboration across clinical, residential, academic, and administrative teams to ensure a supportive, seamless treatment experience. In essence, the Care Manager serves as the “air traffic controller” connecting students and families with program staff and managing activities and communications between all stakeholders.

Requirements

• Must be at least 21 years of age
• Bachelor’s degree in a related field strongly preferred, or 1+ year of experience in a residential treatment or behavioral health setting
• Strong communication, organization, and collaboration skills
• Ability to pass background check

Nice To Haves

• Experience working with families, youth, or clinical teams preferred
• Strong Customer Service experience

Responsibilities

• Act as the primary liaison for parents, school districts, and referents; provide proactive, timely communication and support; coordinate quarterly on-campus family events.
• Coordinate and support Treatment Team Meetings, documentation, and interdisciplinary communication.
• Support new student and family onboarding and ongoing transition processes.
• Provide logistical and administrative support to the Clinical Department and assist with outcomes tracking and documentation.
• Build and maintain trusting relationships with families, internal teams, and external partners; Coordinate campus visits for multiple stakeholders (parents, law enforcement, court officers).
• Ensure completion of bi-weekly surveys for both students and families.
