About The Position

Cayuga Counseling Services is seeking a Care Manager to provide Care Management for eligible Medicaid members under the Medicaid 1115 waiver program. The Care Manager is responsible for providing Care Management services that address the health-related social needs of eligible members under the Medicaid 1115 waiver, which can include housing, nutrition, and transportation needs. This role involves assisting eligible members with securing safe, affordable housing, following up with members to ensure they receive referred services, and referring eligible members to programs within the Network. The position requires strict compliance with HIPAA and all federal, state, and agency confidentiality regulations, policies, and procedures, as well as documenting all services and outreach per program guidelines. The Care Manager will also attend various meetings to educate providers about waiver services and all required trainings. Performs other duties as needed.

Requirements

• Experience with working with Medicaid or vulnerable populations preferred.
• Strong computer skills, including experience with Microsoft Office and electronic documentation systems.
• Knowledge of social services and community resources.
• Strong documentation and organizational skills
• The ability to work independently.

Responsibilities

• Follow up with members to ensure they receive the services they were referred to.
• Refer eligible members to programs within the Network.
• Maintain strict compliance with HIPPA and all federal, state and agency confidentiality regulations, policies and procedures.
• Document all services and outreach per program policies and procedures.
• Attend various meetings to educate providers about the services available under the wavier.
• Attend all required trainings.
• Performs other duties as needed.
